HCBS Settings Individual and Family Survey

Dear Clients and Family members,

The state of Colorado is asking participants of day programs to complete a survey as we work toward state wide compliance with new Medicaid rules. Only those of you who attend via Medicaid or receive Medicaid services need to complete the survey. There are two ways to do so.

1.       Use the following link https://www.research.net/s/ColoradoHCBS

2.       Complete a paper copy and return it to the state. If you want the paper copy we have them at the front desk.

Regardless of the method used please identify KADEP’s address in the survey.
