Updated Paperwork

Paperwork packets will be mailed out this week

Application Update, Release of Information Form, Physician Referral and Adventure Recreation Waiver. This is essential paperwork we need from you - your help in returning them as quickly as possible is greatly appreciated.  Please be thorough in the information you give.  It is extremely important that we have complete information regarding your health status, diagnosis; and especially ALL the medication you are currently taking (even if you do not take meds at the program).  Please note that current standards require that all meds taken at the program, whether administered or not, must be in a labeled medication box.  KADEP can provide med boxes and labels.  Med boxes should be labeled with the following information: participant’s name, medication, dosage, route and time.  It is important that all paperwork be turned in to us by March 9th, 2018.  At this time, participants with incomplete paperwork will not be eligible for any off-campus activities including bowling, hydro, camping, RTD, etc.  If you need help filling out the information, a staff member can assist you.
