2010 KADEP paperwork deadline is approaching


2010 Paperwork is due March 17th 2010!

All KADEP participants:
We need your completed 2010 application, 2010 physician referral, and 2010 release of information forms!

If you need new paperwork, please see Sharon.

Even Sue in the pictures says, "Please turn in your new paperwork!"  :)

It is very important we have updated information on each KADEP participant- Thank you for your cooperation!
