“BE WELL COLORADO”

Chronic Disease Self Management Course

Feel better. Take control of your health.

Stay active and independent.

All patients with neurological conditions and their caregivers are invited and encouraged to take this educational, empowering six week workshop.

Thursdays: 9:30 a.m. to Noon

Be Well Colorado is a six week workshop series designed to help people with ongoing health conditions feel better, live longer and maintain active, independent lives. Workshop leaders teach concrete steps such as goal setting, creating action plans, relaxation techniques, healthy eating and communication skills.

The Be Well Colorado curriculum was developed by Kate Lorig at the Stanford Patient Education Research Center in 1991. Since it was originally developed, this chronic disease management program has expanded and is now taught in more than 20 countries. In 2009, the U.S. Surgeon General endorsed the program.
